With the help of Sarah Swan, Community Relations Specialist, the Centerville Athletic Department has gone digital with their Athletic Forms. The only form that a CCS Student/Athlete has to have physically filled out now is the actual Sports Physical because of the need for a Physician's approval. Here is the process that all Centerville Students and Parents need to follow:
First to fill out the Emergency Medical Form each family will complete the EMA as part of student's Registration for the current school year. Follow these instructions to check whether you've completed the forms for 2024-25 school year, including the EMA needed for Sports:
1.) Log into the Home Access Center. You must login using the parent/guardian's account (not the student's account).
2.) Click on the Registration icon at the top of the page.
3.) Click on the Update Enrollment tab.
4.) Look for the 2024-2025 Returning Student Data Update. If there is a link to Edit, we still need your child’s information for the upcoming school year. If the link says View, you have already submitted the information for the 2024-25 school year.
5.) If your child is a new student to Centerville City Schools, your family completed these forms during the enrollment process and will not have to complete them again until next year when you will have a returning student
Any Athlete that is homeschooled or attends a Private School must do all the paperwork by hand and none of it online. They can come to see us in the Athletic Office and get all that paperwork.
The Concussion, Sudden Cardiac, Code of Conduct and OHSAA Consent Forms are all done on Parent Square. Here is the link:
If you have more than 1 child that participates you must do this process for each child.
In review, all parents of students who attend a Middle School or CHS will do all of their paperwork online except for the physical. If your son/daughter is homeschooled or attends a Private School but plays for Centerville you have to do all of your paperwork the old fashioned way and fill them all out by hand.
